CONDITIONS FOR CONDUCT AND CONTROL OF CHILD

[ ] 1.The child shall not purchase, possess, or consume alcoholic beverages, controlled substances, or tobacco, except as may be prescribed by a licensed physician.

[ ] 2.The child shall attend school as required by law or his parents, guardian or other custodian and obey the reasonable and lawful rules of the school.

[ ] 3.The child shall seek and work faithfully at a suitable employment or faithfully pursue a course of study or vocational training that will equip him for suitable employment; but he shall not change employment without approval of the Juvenile Supervisor or the Juvenile Probation Officer to whom he reports.

[ ] 4.The child shall be home by o'clock p.m. on nights preceding school, otherwise by o'clock p.m., and otherwise shall abide by curfew regulations imposed by ordinance, the Juvenile Supervisor, or the Juvenile Probation Officer, or the child's parents, guardian, or other custodian.

[ ] 5.The child's motor vehicle driving privileges are restricted as follows: .

[ ] 6.The child shall conduct himself in a law-abiding manner at all times and obey the reasonable andlawful commands of his parents, guardian, or other custodian.

[ ] 7.The child shall be subject to supervision by the Juvenile Supervisor and report to him or the Juvenile Probation Officer as may be required from time to time.

[ ] 11.The child consents to permit and to submit his person, place of residence, or vehicle to search and seizure by the Juvenile Supervisor, the Juvenile Probation Officer, or a law enforcement officer, at any time of the day or night, with or without a search warrant, with respect to fruits of crime, contraband, or any article or substance forbidden to the child, and is aware of.

[ ] 12.The child shall submit to a medical examination or other reasonable testing for the purpose of determining his use of alcohol, narcotics, marijuana, or other controlled substances whenever required by the Juvenile Supervisor or the Juvenile Probation Officer.

[ ] 13.The child shall permit the Juvenile Supervisor to the Juvenile Probation Officer to visit him at reasonable times at his home or elsewhere. He shall answer all reasonable inquiries by the Juvenile Supervisor or the Juvenile Probation Officer of any change in address or employment, and report to the Juvenile Supervisor or the Juvenile Probation Officer as directed by either of them.
